99385 would be for the initial physical, new patient 18-39 years of age. 99203-25 would be coded for the abnormality found on the legs and arms with the modifier as required by CPT guidelines to indicate a significant, separately identifiable eval and management service was provided on the same day as the preventative medicine service.
